Request your medical records from your GP

To ensure accuracy and completeness in your Firearms Licence Medical Report, it’s essential to obtain a thorough copy of your GP records. You have the right to request this from your GP practice at no cost. They may provide you with either a paper version or electronic access to your records.

It’s crucial to obtain a complete copy, including all GP consultation notes (including free text) and any relevant letters sent or received about you. Please note that for your Firearms Licence Medical Report, a summary is insufficient. A full copy, including any historical paper or card records, is required.

Below is a sample letter that you can utilize to request your records from your GP.

ELECTRONIC RECORDS

For your convenience, we now accept electronic versions of your GP records. However, it’s imperative that these electronic records are as comprehensive as their paper counterparts. This means they must contain all free text from consultations, as well as copies of any relevant correspondence or letters pertaining to your health.

It’s worth noting that while many GP surgeries utilize computerized record systems like EMIS, they may offer electronic remote access to your notes, typically in a summarized format. If presented with this option, we recommend requesting either full-access electronic records or a printed paper version instead. We understand this process may pose challenges, but it’s crucial for us to certify your notes accurately for submission to the police.

Have the records sent to us
You have the option to submit your records either by traditional mail or by scanning and emailing them to us. If you’re granted electronic access by your GP, you can share this access with us, and we’ll take the necessary steps to ensure that the record is comprehensive and complete.

Pay the fee to us online.
Upon completion of the application form, you will promptly receive a copy of the form and a payment link. Please be aware that these emails may occasionally land in your junk/spam folder. If you encounter any difficulties or have any concerns, please don’t hesitate to contact us for assistance.

MEDICAL CONDITIONS

These are the Medical Conditions we look for that require comment by the police:

  • An acute stress reaction or an acute reaction to stress caused by trauma.
  • Depression or anxiety
  • Suicidal, intention, thoughts, or self-harm.
  • A history or diagnosis of dementia
  • Mania, bipolar disorder, or a psychotic disorder
  • Personality disorders
  • Neurological conditions: e.g. example Multiple Sclerosis, Parkinson’s or Huntington’s disease, or Epilepsy
  • Any alcohol or drug abuse
  • Other mental or physical conditions which might affect the safe possession of a shotgun or firearm.
